A Path to Hope and Housing

Comprehensive programming, including access to wrap-around services, allows individuals to gain the resilience they need to move forward after experiencing houselessness.

With an emphasis on Behavioral Health, our programming meets individuals where they are at - providing comprehensive care that folds together trauma-informed methods, harm reduction, and a strengths-based practice to help individuals meet their personal goals!

INTAKE

Once an individual experiencing houselessness has been admitted for intake at one of our shelters, a Behavioral Health Lead will meet with them to determine their baseline, strengths, interests, and other factors that may determine their path forward.

This information is used to assign the participant a Case Manager who will work with them throughout their time in shelter.

PROGRAMMING

Once settled, participants then meet with their Case Manager to establish their goals for their time in shelter. These can be things such as, but not limited to:

  • Accessing housing

  • Maintaining sobriety

  • Accessing employment and job skills training

Our Wraparound Team works together to engage the participant, connect them to services, and help them toward their goals!

TRANSITION

“Transition” happens when a participant has completed programming and is moving into their next placement beyond our shelters.

If this transition is toward permanent housing, a Mobile Retention Specialist will be assigned to work with this individual to ensure a smooth transition into housing and provide support throughout.

A participant’s final touch-point will be a meeting with the Behavioral Health Lead to gather information about their experiences and sense of resiliency to ensure that we are continuously improving our programming.
